HSE confirm deficiencies in maternity care at Portiuncula Hospital led to two infant deaths and five cases of oxygen deprivation.

On 22nd January 2015, the HSE revealed that quality of care issues had been identified regarding the maternity services provided to seven women and their babies. The review into these cases tragically found that there were two infant mortalities and a further five babies had evidence of oxygen deprivation (hypoxia) during delivery.

Apparent deficiencies in intra-partum care were identified which included interpretation and review of CTG tracings; administration of drugs to accelerate labour; and instrumental delivery.

In light of the initial internal review the families are being advised that their babies' cases will be part of an in-depth review that has been initiated by Portiuncula Hospital Ballinasloe-Saolta University Health Care Group. Details of the review team’s membership and terms of reference have not yet been published.

This is clearly a most worrying development, and raises further concerns regarding the safety of our maternity services following recent high profile reports on maternity units in Ireland.
